Abstract:

Background: It is difficult to treat infections caused by Acinetobacter baumannii due to multiple drug resistance. The aim of this study was to determine the efficacy of combination therapy with two broad-spectrum antibiotics colistin and rifampin on clinical and para clinical indexes of the patients with ventilator-dependent pneumonia due to imipenem-resistant to A. baumannii in intensive care units of a tertiary hospital. Methods: In a descriptive cross-sectional study, fifty patients admitted to the intensive care units in Vali-Asr Hospital, Arak, Iran, from October 2014 to May 2015, with established ventilator-associated pneumonia were subjected to this study and were treated with standard dosage of colistin and rifampin daily. Once the minimum inhibitory concentration (MIC) of the A. baumannii isolates were susceptible to colistin (MIC˂2 µg/ml) and rifampin (MIC˂2 µg/ml) they were treated with three million international units of colistin in 100 ml of normal saline three times a day and 600 mg of rifampin daily. Side effects of combination therapy and variables of patients like fever, blood pressure, white blood cell count, differential count and, mortality were documented and statistically analyzed. Results: 31(62%) patients were improved after combined treatment with colistin and rifampin, 4(8%) patients were died due to bloodstream infection with A. baumannii and 15(30%) passed away in effect of other reasons. The mean of BUN (P=0.07) and creatinine significantly increased (P=0.02). Clinical variables such as fever, white blood cell count, white blood cell percentage and red blood cell sedimentation rate significantly decreased (P≤0.05). Conclusion: Colistin and rifampicin appear to be an effective and safe combination therapy for treatment of ventilator-dependent pneumonia due to imipenem-resistant to A. baumannii

Ventilator Associated Pneumonia by Acinetobacter baumannii And antimicrobial susceptibility pattern

Background: Ventilator Associated Pneumonia (VAP) is one of the serious complications of ventilator support occurring in ICUs. The aim of this study was to determine the susceptibility pattern of Acinetobacter baumannii (A. baumannii) isolated from patients with VAP by two methods: disk diffusion and E. test. Methods: This descriptive-cross sectional study was conducted in the four ICUs of Ras...

Antibiotic combinations for serious infections caused by carbapenem-resistant Acinetobacter baumannii in a mouse pneumonia model.

OBJECTIVES Successful therapy of carbapenem-resistant Acinetobacter baumannii strains has been reported with colistin, but recently we argued against its use as monotherapy because of the poor results obtained in a mouse pneumonia model. Our aim was to identify antibiotic combinations that were valid therapeutic alternatives in the same model. METHODS We used two carbapenem-resistant A. bauma...

Carbapenem-resistant Acinetobacter baumannii

To the editor: We read with interest the paper of Hrabák et al. [1] on the isolation of a blaNDM-1-carrying Acinetobacter baumannii strain. However, although the authors believed to be the first in the Czech Republic to report on a New Delhi metallo-beta-lactamase-1 (NDM-1)-producing bacterium, there have been other reports on the same A. baumannii strain in the country [2,3]. Here, we would li...

Tea as a source of Acinetobacter baumannii ventilator-associated pneumonia?

To the Editor: Acinetobacter baumannii increasingly has been involved as an agent of hospital outbreaks worldwide. The most common site of nosocomial infections is the respiratory tract, especially in ventilated patients, followed by bloodstream infections. The incidence of Acinetobacter species as a cause of nosocomial pneumonia has increased despite advances in the management of ventilated pa...

Prognostic factors associated with mortality of drug-resistant Acinetobacter baumannii ventilator-associated pneumonia

BACKGROUND Ventilator-associated pneumonia (VAP) caused by drug-resistant Acinetobacter baumannii is associated with high mortality in critically ill patients. We identified the prognostic factors of 30-day mortality in patients with VAP caused by drug-resistant A. baumannii and compared survival outcomes among multidrug-resistant (MDR), extensively drug-resistant (XDR) and pandrug-resistant (P...

Colistin Dosage without Loading Dose Is Efficacious when Treating Carbapenem-Resistant Acinetobacter baumannii Ventilator-Associated Pneumonia Caused by Strains with High Susceptibility to Colistin

OBJECTIVES This study aims to analyze the mortality and the length of ICU stay (LOS) of A. baumannii VAP compared to respiratory colonization in patients with mechanical ventilation (MV). METHODS A prospective cohort study was performed in an ICU of adult patients (February 2010-June 2011). One hundred patients on MV with A. baumannii in lower respiratory airways were recruited, and classifie...
